_*POSITION SUMMARY*_
We Care of Central Florida is a non-profit organization in Polk County, Florida, that provides FREE access to specialized healthcare services, labs/diagnostics, breast care, vision care, transportation, and durable medical equipment to uninsured, financially struggling residents of Polk County.
Project Helpful Hearts’ mission is to collect new and gently used durable medical equipment (DME) to support those who are mobility impaired due to physical disability, illness, or injury.
Through this project, donated DME is then loaned to adults, children, and caregivers so they can live, work, and play regardless of their physical challenges.
Reporting directly to the Operations Director (OD), the Project Helpful Hearts Coordinator (PHHC) works one-on-one with referred patients seeking access to free, durable medical equipment and donors wishing to donate new and/or gently used DME.
_*QUALIFICATIONS*_
Demonstrate telephone, computer, and written communication skills
Ability to communicate effectively with people of all socioeconomic and situational backgrounds
Ability to prioritize tasks and document visits
Ability to work indoors and outdoors in hot and/or cold weather
Ability to work with cleaning supplies/disinfectants
Ability to provide optimal customer service, including acting as a patient advocate, utilizing a patient-centered approach
Be able to follow verbal as well as written instructions
Be able to speak, write, and read English proficiently
Be able to multi-task and prioritize work
Have a professional personal presentation and demeanor
Be able to pick up and move 40 pounds safely
Must have and maintain a clean Florida driver’s license
Willing to undergo a background check
_*ROLES AND RESPONSIBILITIES*_

*Storage Management (3 locations)*
Maintains, documents, and organizes the inventory in storage so it is fully accessible
Maintains all storage areas so it is kept as clean as possible and it can be presented to donors and potential donors
Maintains storage areas so it is kept as safe as possible for all guests, staff, and volunteers
Supervises storage area staff and volunteers as requested and/or required
Picks up DME and other equipment and supplies from donors
Ensures receipts for DME received are filled out correctly and verified for accuracy
Handles questions from donors, clients, and potential donors and clients in a timely, professional, and respectful manner
Provides support to WCCF staff as needed and requested and as is physically possible
*Provision of DME*
Cleans and maintains all DME as needed
Becomes thoroughly familiar with the primary use of all DME equipment maintained in the warehouse
Ensures all patients receive manufacturer instructions with each piece of DME provided
Ensures all patients receiving DME have been deemed eligible per the County/State guidelines
Operates equipment such as electric beds, motorized wheelchairs, scooters, and other equipment
Pick-up/Deliver DME in an issued vehicle
Ensures appointments for DME and equipment pickup/drop-off are met as per the times agreed upon with the patient and/or donor
*Intake/Eligibility*
Determine patient eligibility and accurately complete the required documentation for enrollment per State and County guidelines
Coordinate overall online and in-person intake, assessment, and eligibility process
Provide clear guidance and expectations of patient program participation
Demonstrate, maintain, and communicate programmatic policies and procedures
Input data collection and update all progress in program-designated data tracking systems
Generate required reports
Obtain, upload, and record all necessary patient demographics and documentation in specified databases
Ensure that positive outcomes and goals are met as required by the organization
Work with the team on implementing processes that increase positive customer engagement
*Other Tasks*
Back-up in office as needed (phone overflow, lunch break & days out)
Operate standard office equipment, such as computers, copiers, phones, and fax machine
Keep information confidential under HIPAA
Maintain high ethical standards and integrity
Other duties assigned by CEO/Operations Director
Provides support with special events, fundraisers, and other tasks and special projects as assigned
